we need to add medical form with fillable information to our online registration. Can you help me with that?

Hi back with another question, I am working on scheduling, and I have 4 teams or groups but it adds a 5th group when I look at the schedule how do I eliminate that group.  Also, we have a free time slot in our different stations that is intentional.

I will be doing online registration but I wanted the paper form to be ok if it was printed out to use.

Thank you for your help. I did as you suggested to put the two medical components in separately, but they still run together. Is there a way to put a space between them?
