Permission Form
Our students, entering 5th grade, need an extra permission form because we take them off-site. Is there a way to add a permission form to the registration process?
Answer
If all you need is for parents to check a box giving permission, you can customize the registration form to include that. You'd need to include a note that this only applies to 5th grade.
If you need a permission slip filled out, you could add that information to the email they receive after registering, and include a link to a form they can print out and bring in. You'd need to create a form and put it in a publicly accessible place on the internet.

Can I include links or attachments in the registration email?
Hi Brad, thanks for your quick response! How do I customize the student form to include a check box and appropriate text?
I have the permission checkbox figured out, but our team is wondering if there is capability for an electronic signature? I'm assuming the answer is not, but just checking :-) Thanks!
No. You can do a "click to agree" checkbox, or a "Type your name in the field" on the registration forms, but we don't support any type of more secure cryptographic electronic signature.
Dear Spring:
If this can help ... This is what we've done the last three years on VBS Manager, etc. We use verbiage in our Parental Consent Form/Agreement section such as:
"As the Parent(s) or Guardian, I understand that the child/children I am registering (either online or in person) will be under the direct supervision of the adult sponsors of... As the Parent(s) or Guardian, I give my full permission for him/her to attend the Vacation Bible School (VBS) program that is sponsored by ... Also, by adding my name in this box, it will serve as my electronic signature. As the Parent(s) or Guardian, I hereby give consent to the administration of any medical treatment which may be necessary in the opinion of a licensed physician or medical ... ." you get the picture...
Brad, how do I access the option to "Type your name in a field?" I'm not seeing that as an option...
Sorry I wasn't clear. I meant that you can create a "Simple Text" field and give it a label like "Type your name in the field to indicate your consent"
